Fill Centers USA is actively recruiting and training Aftercare Providers. If you find that we don’t have a location near you, there are a couple of things you can do.
- Submit a Location Request Form to us so we can recruit and train a qualified medical professional in your area. You will receive regular updates, in your email, with the names and locations of our new Aftercare Providers.
- Give us the name of your Primary Care Provider, or another medical professional in your area that you trust, and we’ll train that professional to be the Aftercare Provider in your area. Click here for Aftercare Provider Nomination Form.
*Note: Please, find out if your Primary Care Provider would be interested in being trained prior to filling out this form. Also, the medical professional you recommend must meet our pre-screening qualifications to be accepted for training.
